Passed through 1st/2nd and finally 3rd level support - by the time the problem comes to 3rd line its major - and we have no documentation/knowledge of how the server has been configured due to lack of documentation/not being involved in the final stages of the build process. 2nd line are doing 3rd line work and nothing comes through to 3rd line.
