Cox support says it is probably the firmware. That firmware was released in April of 2016 to prevent reset through malware, to the modem defaults. I don't buy that answer since there are multiple installations of this modem in the Cox network. It seems to start with Upstream channels dropping out. Here is what my modem looks like:
