First: I don't think you must fill out these forms online. In fact I think it would be better if you downloaded the form to your local disk, and fill / print them from there. Second, I do not understand how your font pack download could possibly open Adobe Reader.

I think you may find that if you wanted the Spanish version of Captivate all you had to do was choose Spanish as the language from the dropdown menu when you installed it. Captivate doesn't have "language packs" like some other software that enables you to flick a switch and decide at will which language it uses. It's decided at install.
