To begin the CSG program, interested persons must attend the monthly in person CSG orientation at the Northern Service Center or complete this electronic orientation. Grant amounts are based on home care assessments and ratings for home health aide, private duty nursing or personal care assistant services. Consumers can get an estimate of their grant from their county coordinator or the DHS Consumer Support Grant coordinator. Consumers will work with their case manager to develop a service plan.

Families can contact their County social worker to request an application. Once returned, the application will be reviewed and scored based upon need. The Consumer Support Grant (CSG) allows a person who has Medicaid Home Health Aide (HHA), Personal Care Assistant (PCA) and Home Care Nursing (HCN) services to receive a cash grant to purchase supports and services. The person, as the employer, may use the CSG to hire the supports they need of their choice.

B. Benefits of the Consumer Support Grant Program Persons of any age or disability who have a functional limitation can benefit from the Consumer Support Grant Program. The grant is intended to replace services the consumer is currently receiving such as homemaker, personal care attendant, and home health aid services.
