This is in response to 4h emergency response or 72h response referrals from the GP. DIST also receive referrals from the CMHT to provide intensive support to promote admission avoidance.

Once your referral is forwarded to the Dementia and Complexity in Later Life (DCLL) Dementia and Intensive Support Team, they will organise a time and date to meet with you. At this session an assessment is undertaken. From this assessment a plan of action will be agreed with you.
