Horizons Specialized Services was created in 1975 to provide services to individuals with developmental disabilities in five northwest counties of Colorado: Grand, Jackson, Moffat, Rio Blanco and Routt. Once a family is permanently housed, we work with parents to support their long-term success, so that the cycle of homelessness can be permanently broken.
