This is almost definitely caused by the underlying system not having FreeType installed (if FreeType is already installed then @user3338098's solution should fix it).. If you come across this problem absolutely do not follow the most upvoted answer (@nacholibre's). Either install the freetype library on your server (usually freetype, libfreetype or similar, will vary depending on distro.

Jul 08, 2011 · On our D6 installation we have CAPTCHA 6.x-2.4 plus Captcha Riddler 6.x-1.1 and Image CAPTCH 6.X-2.4 (all from the same package I believe). All three are installed and enabled. Although we have used this module before with success on other sites, it is not working on this site which is a subdomain on our staging server.

Here's the reply from my host: I've verified that Opentype is enabled in the GD library, so utilizing captcha with true type fonts should be working fine, though I believe you must have the actual 'TTF' file downloaded to the server and available to the captcha script.
