Every person with a diagnosis of dementia in Scotland is entitled to one year of Post Diagnostic Support. The 5 Pillars Model provides a framework for people living with dementia, their families and carers to live as well as possible with dementia and prepare for the future.

8,178 people were referred for dementia post-diagnostic support (PDS) in 2016/17. This equates to 46.7% of people estimated to be newly diagnosed with dementia in Scotland in 2016/17. Of those people newly diagnosed with dementia during 2016/17 who were referred for post-diagnostic support (PDS), 83.9% (6,830/8,145) received one year's support.
