The current sign_place() function is basically mimicking ":sign place". One of the quircks is that it takes a file name, not a buffer number. It also has an optional {id}, where zero means it's not passed in. Adding a new function that takes a list of dictionaries could be a good alternative.

Microsystem's dtterm does support this it's the only terminal emulator I know of which does). A text sign and the highlight should be feasible in any color terminal emulator. Signs and highlights are not useful just for debuggers. Sun's Visual WorkShop uses signs and highlights to mark build errors and SourceBrowser hits.
