The main change is to define the SPI select of the WIZ5500 Ethernet chip and set it high (unselected) so the sketch can talk to the SD card only. For the Ethernet shield, put your formatted SD card into the SD card slot (top side up, don't force it, gentle). Put your shield on your Arduino-compatible board.
