This message might have been caused by using PHP 7 and choosing "mysql" in installer: "PHP Support for database is not enabled." The "mysql" extension has been removed in PHP 7. You have to choose "mysqli" if you want to run 1.2 with it.

Sep 21, 2018 · Maybe your provider changed your PHP version to a newer one (>= 7.0). Maybe there is a setting where you can change it back. If not, you have to upgrade Mantis, as Mantis 1.2.5 does not support latest PHP versions.

Then it all worked, except that for some strange reason, the php.ini file that gets picked up is the one from my old PHP directory, not the latest upgrade one. But this can be detected by running phpinfo which shows the loaded php.ini file so I knew which one to amend with the fastcgi options.
